1 – Complete the Request for services form and send it by email to centredejour@maisonstraphael.org

2 – The director of multidisciplinary services will contact you to organize an initial assessment and the choice of activities.

Admission criteria for the Day Centre

For people living with an incurable illness

Be 18 years of age and over.
Have been diagnosed with an incurable disease limiting life expectancy.
Live or have a caregiver who lives in Montreal.
Living at home.

For caregivers and bereaved

Be the loved one of a person diagnosed with an incurable disease limiting life expectancy.
Live on the territory of Montreal, or be the loved one of a person diagnosed with an incurable illness limiting life expectancy who resides there.

We will welcome people from all backgrounds, regardless of their social status, ethnicity, sexual orientation and gender identity or religious beliefs.
