Request for services at Maison St-Raphaël

Admission to the Care Home

Maison St-Raphaël provides care to end-of-life patients. Since we are not a long-term care facility, we must screen patients to only admit those who meet the criteria for the care we provide. Care is specifically aimed at patients in the terminal stages of their illness and who require their symptoms to be managed by an interdisciplinary team of palliative care experts.

Step 1

Ask your healthcare team to complete the Palliative Care Home Service Request form and send it to admission@maisonstraphael.org. Make sure the form is signed by the patient’s treating physician.

Step 2

A member of the Maison St-Raphaël care team will evaluate the file and contact the patient’s care team within 72 hours to provide a response.

Step 3

If the request is accepted, the admission time may vary from a few days to a few weeks, depending on care needs, prognosis, presence of symptoms and bed availability.

Admission criteria to the Care Home*

  • Patients and/or primary caregiver must reside on the island of Montreal
  • Must have a medically recognized life expectancy of three months or less
  • Must be at least 18 years old and registered with the Régie de l’assurance maladie du Québec (RAMQ)
  • Have an incurable illness, limiting life expectancy
  • Have stopped any systemic treatment requiring monitoring

Registration at the Day Centre

A Day Centre for everyone, regardless of their social status, ethnic background, sexual orientation and gender identity, or religious beliefs.

Step 1

Complete the Request for services form and send it by email to centredejour@maisonstraphael.org

Step 2

The director of multidisciplinary services will contact you to organize an initial assessment and the choice of activities.

Admission criteria to the Day Centre*

For people living with an incurable illness

  • Be 18 years of age and over;
  • Have been diagnosed with an incurable disease limiting life expectancy;
  • Live or have a caregiver who lives in Montreal;
  • Living at home.

For caregivers and bereaved

  • Be the loved one of a person diagnosed with an incurable disease limiting life expectancy.
  • Live on the territory of Montreal, or be the loved one of a person diagnosed with an incurable illness limiting life expectancy who resides there.

We will welcome people from all backgrounds, regardless of their social status, ethnicity, sexual orientation and gender identity or religious beliefs.
