Process to request an admission to St. Raphael’s
St. Raphael’s provides comfort care to patients at the end of their lives. Since we are not a long-term care facility, we must carefully screen patients in order to admit only those best suited for the care we provide. The care is specifically geared towards patients who are in the terminal phase of their illness and who require their symptoms to be managed by an interdisciplinary team of palliative care experts.
STEP 1
Ask your health care team to complete the Request for Service form for St. Raphael’s and send it to admission@maisonstraphael.org. Make sure the form is signed by the patient’s attending physician.
STEP 2
A member of the St. Raphael’s healthcare team will evaluate the file and contact the patient’s healthcare team within 72 hours to provide a response.
STEP 3
If the request is accepted, the admission time may vary from a few days to a few weeks, depending on care needs, prognosis, presence of symptoms and availability of beds.
Admission criteria for the home*
- The patient and/or the primary caregiver must reside on the Island of Montreal
- Must have a medically supported life expectancy of three months or less
- Must be at least 18 years old and be registered with the Régie de l’assurance maladie du Québec (RAMQ)
- Must have a documented, progressive, life-limiting and incurable illness
- No longer be receiving any systemic treatment that requires monitoring
